Self Assessment Form is a tool for study program management team to measure study program operational management in providing quality assurance, compliant to government regulations and therefore readiness for accreditation.

The self assessment for academic scope is grouped into 6 (six) aspects, based on 9 (nine) criteria of national accreditation body matrix. For each and every aspect study program management team should answer all questions and provide link to the required documents for reference.
